Proxy Access
How to give another person access to your GP record.
Proxy Access
What is Proxy Access?
Proxy Access is when patients choose to give another person access to their online GP record. This is usually a family member, next of kin or a carer.
A Proxy may be chosen for many reasons, including:
Providing better information to family members and carers about your medical conditions
Giving someone access to your prescriptions record, so they can understand what medications you need and how much
To set up a proxy at Knightwick, you will be asked to complete a form providing details of your desired proxy. Once this is returned to the Surgery, we will activate the proxy access.
